Date of Application

• Individual or dependents shall apply for rejoining from the date of return to the country

Certificate Enclosed

• NHI Rejoining Application Form (including the entry/exit certificate issued by the Immigration Office)

Notes

• When the insured leaves the country for longer than 6 months, the insured should apply for rejoining from the date of return to the country and start paying premiums. When the insured leaves the country for a period not longer than 6 months, the insured should apply for rejoining from the date of suspension and settle all outstanding premiums.
